ST. JOSEPH — The Michigan Health Endowment Fund has announced that the Region IV Area Agency on Aging, in partnership with Spectrum Health Lakeland, will receive a $500,000 grant for implementation of the Southwest Michigan Care Continuum Transformation Strategy, according to a news release.

This strategy seeks to build an infrastructure in which older adults with complex care needs are linked with proper medical and community-based services that integrate social care into the delivery of health care.
